Deep beneath the ruins of a near-future Eastern European metropolis, the old metro system has become the last sanctuary for survivors of a devastating viral outbreak known simply as “Z.” The surface is controlled by infected swarms—fast, aggressive creatures whose humanity has long dissolved. In the dim tunnels, scattered communities struggle to keep order, relying on scavengers to bring back food, medicine, and information from the world above. Among these survivors is Arsen Markovic, a former civil engineer who knows the metro better than anyone alive.
Arsen lives with a small group stationed near the abandoned Line 7, an area rumored to be unstable and haunted by infected that slipped underground. When a young girl named Mila goes missing during a supply run, the settlement begins to panic. Mila carries a rare immunity that scientists once believed could lead to a cure, and losing her could mean losing hope entirely. Arsen volunteers to track her down, driven by guilt over a past failure he has never spoken about.

Venturing deeper into forbidden sections of the metro, Arsen discovers signs of a hidden faction known as the Black Lanterns, a rogue group that rejects any attempts to cure the infection. They believe the world above deserved its fate and seek to control what remains of humanity in the tunnels. Rumors claim they are experimenting with the virus, trying to harness the infected as weapons. Arsen soon realizes Mila has been taken by them, not by the creatures that roam the dark.
As he pushes forward, Arsen encounters infected that behave differently—faster, smarter, almost coordinated. Tracks and abandoned stations become battlegrounds as he navigates traps set by human enemies and ambushes from mutated swarms. Along the way, he forms an alliance with Irena, a former soldier whose squad vanished after a failed mission in the same area. She knows the Black Lanterns’ tactics, but she also carries secrets that make Arsen question whether he can fully trust her.
Together, they infiltrate an underground research hub where Mila is being held. The Black Lanterns plan to release a modified strain of the virus into the lower tunnels, eliminating rival settlements and establishing dominance. Arsen and Irena race against time, battling both the infected and the faction’s zealots in collapsing corridors filled with toxic chemicals and broken machinery.
In the final confrontation, Arsen must face the truth he’s avoided for years—that his engineering work once contributed to the metro’s downfall during the outbreak’s early days. Saving Mila becomes not only a mission but a chance at redemption. The escape triggers a chain reaction that floods the Black Lanterns’ base, trapping the infected and sealing off the most dangerous part of the metro.
